What is the declaration form of change to residence information - Form CT01 issued with Circular 66/2023/TT-BCA?

Under Article 4 of Circular 66/2023/TT-BCA:

Forms
This Circular is promulgated together with the following forms:
1. Declaration of change to residence information, coded CT01 and replacing Form CT01 issued together with Circular No. 56/2021/TT-BCA.

Thus, Form CT01 issued with Circular 66/2023/TT-BCA is the latest declaration form of change to residence information in 2024 as follows:

The instructions for preparing the declaration of change to residence information made in Form CT01 in Vietnam are specified as follows:

(1) Dear and personal information:

*Dear: Name of residence registration authority.

*Personal information

- Family name, middle name, and first name:

Write in uppercase or lowercase letters, with sufficient accents (write the correct family name, middle name and first name indicated on the birth certificate) of the applicant.

Example: NGUYEN VAN A

- Date, month, year of birth:

Record the date, month and year of birth according to the calendar year and match the birth certificate of the person who has changed residence information. Note: write 02 digits for date of birth, 02 digits for months of birth are months 01 and 02, and 04 digits for year of birth.

Example: December 18, 2001

- Gender:

Include the gender of the person whose residence information changed as "Male" or "Female".

- Personal identification number:

Write down your full personal identification number (citizen ID card number).

- Contact number:

Write down the number of the mobile phone or desk phone currently in use.

- Email:

Write down the email address to contact (Optional, if any, write)

- Family name, middle name and first name of head of household:

Write in uppercase or lowercase letters, with sufficient accents (write the correct family name, middle name and first name indicated on the birth certificate) of the head of household.

- Relationship with the head of household:

+ In case of permanent or temporary residence registration while having a lawful place of residence or house under his/her ownership or being leased by the landlord:

Write as the head of the household, ie register yourself as the head of the household.

+ In case of permanent or temporary residence registration in case the head of household allows to add to the household or temporarily reside:

Record the actual relationship with the head of the household. For example, Wife, biological child, son-in-law, biological grandchild or person who stays, borrows, rents, stays together, rents together, stays together...

+ In case of change, verification of residence information (for example: change of residence; correction of personal information; separation of households; deregistration of permanent or temporary residence registration or verification of previous permanent residence registration...)

Record the relationship with the head of household according to the information declared in the national database on residence or record according to the household registration book or temporary residence book.

- Personal identification number of the head of household:

Write down the full personal identification number (citizen identification number) of the head of the household.

- Household members who change together: Fill in the same information as the head of household.

(2) Contents of the request:

Clearly and specifically write the content of the request.

For example, permanent residence registration; temporary residence registration; separation of households; and verification of residency information...

(3) Opinions of the head of household:

Applicable to the cases specified in Clauses 2, 3, 5, 6, Article 20; Clause 1 of Article 25; Point a, Clause 1, Article 26 of the Law on Residence 2020.

The collection of opinions of the head of household is carried out by the following methods:

- The head of household clearly writes the consent content and signs and clearly writes his full name on the declaration.

- The head of household confirms the content of consent through the VNeID application or other online public services.

- The head of the household has a separate document clearly stating the consent (this document is not notarized or authenticated).

(4) The opinions of the owner of the lawful place of residence

Applicable to the cases specified in Clauses 2, 3, 4, 5, 6, Article 20; Clause 1, Article 25 of the Law on Residence 2020; Point a, Clause 1, Article 26 of the Law on Residence 2020.

The collection of opinions of the owner of legal accommodation shall be carried out by the following methods:

- The owner of the legal accommodation clearly states the consent content and signs and clearly states his or her full name on the declaration.

- The owner of the legal accommodation confirms the content of consent through the VNeID application or other online public services.

- The owner of the legal residence has its own document stating the content of consent (this document is not notarized or authenticated).

Note: In case the lawful place of residence owner consists of many individuals or organizations, the consent of all co-owners must be obtained, unless there is an agreement on appointing a representative with consent; In case the lawful place of residence owner confirms the content of consent through the VNeID application, the citizen must declare information about the family name, middle name and first name and registration number of the owner of lawful place of residence.

(5) Opinions of a parent or guardian:

Applicable to cases where minors, people with limited civil acts, people who are incompetent for civil acts have changed their residence information.

Parents or guardians give opinions by the following methods:

- The parent or guardian clearly writes the consent content and signs and clearly states the full name on the declaration.

- The parent or guardian confirms the content of consent through the VNeID application or other online public services.

- The parent or guardian has their own written consent (this document is not notarized or authenticated).

(6) Declarant:

In case of direct submission, the declarant signs and clearly states the family name, middle name, and first name in the declaration.

In case of submitting through the public service portal or VNeID application, the declarant does not have to sign this section.

(7) Personal identification number:

Only declare information when citizens request to confirm consent content through the VNeID application.

What are the cases temporarily ineligible for change of place of residence in Vietnam?

According to Article 5 of Circular 55/2021/TT-BCA, the cases temporarily ineligible for change of place of residence in Vietnam include:

Case 1: Persons mentioned in Points a, b and c Clause 2 Article 4 of the Residence Law shall be temporarily ineligible to change their places of residence while their right to freedom of residence is restricted unless the competent authority gives the permission for such change in writing and it falls under the case provided for in Case 3:

- Persons banned from leaving their place of residence or temporarily detained by presiding officers or presiding authorities; convicts yet to receive the sentence enforcement decision or having received the sentence enforcement decision but out on bail or granted suspension of sentence enforcement; convicts undergoing probation;

- Persons serving a custodial sentence, imposed a residence ban, under mandatory supervision or serving a community sentence; and persons on parole;

- Persons currently subject to compulsory education in their communes or serving a sentence in a correctional institution or rehabilitation center;

- Persons to serve a sentence in a correctional institution or rehabilitation center but granted suspension of sentence enforcement; and

- Persons under supervision during consideration of admission to a correctional institution or rehabilitation center;

- Persons placed under medical quarantine as they pose the risk of spreading an infectious disease to the community;

Case 2: Persons living at locations or in areas currently placed under quarantine for epidemic prevention and control or due to declaration of state of emergency by the competent authority or person shall be temporarily ineligible to change their places of residence from the start to the end of such quarantine measure.

Case 3: Persons currently imposed a residence ban shall be temporarily ineligible to register locations where they are banned from residing as their places of residence until the ban is fully served or they obtain written permission from the authority imposing the ban.

What are the forms of notifying the results of residence information declaration in Vietnam?

Under Clause 4, Article 3 of Circular 55/2021/TT-BCA amended by Clause 1, Article 1 of Circular 66/2023/TT-BCA:
